My last clinical experience was at TOSH: The Orthopedic Specialty Hospital. I worked on the acute inpatient floor. I had a great time with this experience. The patients that I saw each day were people who had undergone major joint surgery, usually total joint replacement. The majority of the patients had a total knee or hip and our goal was to get them up out of bed as soon as possible and then to have them trained on assistive devices for ambulation and stairs so they could return home and continue to be functional within their home. Along with TKA and THA, I was able to see spinal decompressions and fusions, total ankle replacements, total shoulders and reverse shoulder replacements. Many of our patients would leave the day after surgery or 2 days after surgery. I was able to get patients started on exercises that would help reduce pain and assist them in getting into and out of bed or to maintain a functional range of motion. I spent a great amount of time with each patient on education. I taught them about healing, affects of overuse and inflammation, edema management, and safety. I had many opportunities to see some of these patients in the outpatient clinic when my patient case load was complete each day on the inpatient floor, so I could follow how they progressed weeks after their replacements. I was also able to observe the surgeries to give me a better appreciation for the surgery and to be able to better understand what the surgeon did and how to help the patient understand what we were having them do during their therapy sessions. I learned about procedures I had not heard of and I chose to complete a case study on one that I wanted to learn more about. Attached is my case study on a patient who had a reverse shoulder arthroplasty done. I also was asked by my CI to complete an in-service before I completed my clinical rotation.
